Students will submit a report that summarizes the results of a community health assessment utilizing the social-ecological model to examine the social determinants of health of UAB SHARP patients. This paper will be supported by scholarly evidence and will demonstrate the student\u2019s acquisition of graduate-level public health concentration knowledge and skills in addressing health disparities and the social determinants of health. You should tailor your report for UAB leadership in public health and social service as well as medicine and nursing. In your report, you will also demonstrate how your public health concentration competencies can be useful for key stakeholders in the community health assessments process<\/p>\n
