\nThe Doomore Hospital, Primicare Clinic, and MaxLab all operate in
\nSmalltown, New Hampshire. Doomore hospital has had an EHR for 7 years.
\nPrimicare Clinic has just implemented its clinic-based EHR in the past
\nyear. Both of these entities need to send lab orders to MaxLab, and
\nobtain the results back quickly and accurately. Primicare needs to send
\nprimary care and maternity patient data on drug allergies and patient
\ncare summaries to Doomore Hospital, when needed.<\/p>\nDoomore has a different patient demographics and patient ID system
\nin its EHR, than Primicare\u2019s system. The patient ID for Doomore, is not
\nthe same as for Primicare\u2019s ID. Also different, are each entity\u2019s drug
\nallergy tables\/ fields, which are represented in the tables below.<\/p>\n
Also, for its patients discharged from Doomore\u2019s ED and inpatient
\nfacility, Primicare wants to review the hospital records promptly.
\nPrimicare wants at least, the patient\u2019s full discharge medication list,
\nall nursing and physician care summaries, and Discharge Orders. This
\nwill allow the primary care practitioners to better follow up on their
\npatients\u2019 care and compliance.<\/p>\n
